members, and the entire health plan population. That’s our business
and it’s the only thing we do.
OUR INTERVENTION PROCESS
Care Assessment
Our members are not print-outs or claims. They are unique individuals
whom we get to know on a personal level. We realize a lot can
change with a member’s health status in the 60 to 90 days
after the last claim was filed. That’s why we’ve
added an additional identification tool; our individualized
Care Assessment, which only AirLogix provides.
We first identify members that may need assistance with their
respiratory disease, and then we call that member to determine
what level of support is needed -- not the other way around.
Our specialized respiratory staff ask members a series of
specific clinical and demographical questions to determine
the appropriate level of support. Our experience has shown
this assessment is particularly valuable in the stratification
of respiratory diseases because we can hear the member speaking
and breathing over the phone and are able to ask questions
based on that discovery process, not just on claims paperwork.
Education and Access
All members are given a copy of our award-winning Self- Management
Guides. Individualized for asthma and COPD members, the guides
describe an asthma or COPD episode and define early warning
signs. They educate members about methods to identify and avoid
triggers, encourage positive behavior changes and provide comprehensive
medication information.
We also provide members with detailed information on developing
a self-care plan and why it’s important for their ongoing
health. Finally, a self-care kit is provided to members with
a peak flow meter (for asthma members), a spacer for metered
dose inhaler (MDI), and an instructional video on how to use
them.
